The Role of the Instituto Social de la Marina (ISM)

In Spain, maritime medical examinations (Reconocimiento Médico de Embarque Marítimo) are officially regulated and managed by the Instituto Social de la Marina (ISM), which operates under the Ministry of Inclusion, Social Security and Migration. To get a certificate recognized by the Spanish maritime authorities, you must be examined by an ISM-approved doctor at an authorized center.

Where to Get the Examination in Alicante and Torrevieja

While the ISM has its own provincial directorates, they also authorize private clinics to conduct these specific examinations. It’s crucial to ensure the clinic is explicitly approved for maritime medicals, not just general health checks.

  • Alicante City: The primary point of contact is the ISM Provincial Directorate in Alicante. They can direct you to their in-house doctors or affiliated private centers such as specific branches of the Quirónsalud network or the Igualatorio Médico Quirúrgico.
  • Torrevieja: In the Torrevieja area, clinics like the Centro Médico Parque de las Naciones or specific specialists within the IMED hospital network may hold the necessary ISM authorization. Always call ahead to confirm their current status as an approved maritime medical examiner.

Mandatory Tests and Medical Standards (ISM/ILO)

The examination is comprehensive and designed to ensure you will not pose a danger to yourself, others, or the safe operation of the ship. The standard check-up includes:

  1. Physical Examination: General health, BMI, cardiovascular check (ECG), and respiratory function.
  2. Vision and Color Blindness: Strict standards apply, especially for deck officers. Tests include visual acuity (with and without glasses) and color vision (e.g., Ishihara plates).
  3. Hearing Test (Audiometry): Essential for engine room personnel and officers communicating via radio.

Costs and Validity of the Certificate

The cost of a seafarer’s medical examination at a private authorized clinic in Spain generally ranges from 80 € to 120 €. If you are actively contributing to the Spanish social security system under the special maritime regime (Régimen Especial del Mar), the examination at an ISM center might be free or subsidized.

According to STCW regulations, the medical certificate is valid for a maximum of 2 years for seafarers over 18 years old. For seafarers under 18, it is valid for 1 year. If your certificate expires during a voyage, it may remain valid until the next port of call where an approved medical practitioner is available, provided this period does not exceed three months.
